This book explains that folate (B9) and vitamin B12 deficiency—disrupting one-carbon metabolism—underlies both vascular and developmental diseases that rarely appear in the same clinical context. The retina offers a unique diagnostic solution: as accessible neurovascular tissue that mirrors brain microvasculature, it can reveal metabolic dysfunction through non-invasive imaging before advanced disease develops. Part I establishes the scientific foundation: the retina's embryological origin as brain tissue, the biochemistry linking B9/B12 to endothelial health and DNA methylation, and how retinal vascular changes serve as proxies for systemic microvascular status. Two key papers demonstrate how retinal imaging—read by clinicians or AI—can detect cerebral folate deficiency and other metabolic disruptions that would otherwise require invasive testing.
